In this article, Vamvakas and Blajchman, in a “tour de force”, reviewed the causes of allogeneic transfusion-related mortality as documented in more than 300 publications! The three leading causes in the US today are transfusion-related acute lung injury (TRALI), transfusion-associated sepsis (TAS), and hemolytic transfusion reactions (HTR). During the last decade, TAS- and HTR-related deaths have decreased but TRALI has been on the increase. The authors propose six possible strategies for reducing transfusion-related mortality, and they conclude that although zero risk is not achievable significant further reduction in transfusion-related mortality is feasible and should be pursued. An excellent review that should be read by all!
